Questionnaire: What is your Experience of Care Provided in the Community? We are in the middle of a great change in the way that Health and Social Care is delivered locally. The idea is that hospital beds should be reduced and where possible people should be cared for in their own homes – Community based teams should be developed to avoid people being admitted to hospital. Where people do have to be admitted support should be there in the Community to ensure that they can be discharged as quickly as possible. Sometimes care need only be for a short period – sometimes it needs to be on a continuous basis. What is your recent experience of care provided in the Community?
