INTERMEDIATE CARE - the Third Way to deliver care to patients,  in their own homes

The NHS is modernising the way it delivers its services.
Community Integrated Intermediate Services (CIIS) or Intermediate Care, is midway between GP care and going into hospital.
For suitable patients, a bespoke package of care is delivered in their own homes by a range of healthcare professionals.
