Mae Bwrdd Iechyd Hywel Dda yn edrych ar y ffordd mae gwasanaethau gofal iechyd yn cael eu darparu i bobl o fewn ein cymunedau.
Mae’r Fframwaith Pum Mlynedd drafft yn egluro ein gweledigaeth i ddarparu gwasanaethau wrth galon eich cymuned leol gan ymateb yn uniongyrchol i’r heriau sydd wedi’u nodi yng Nghynllun Iechyd Gwledig Cymru.
Er mwyn cyflawni’r weledigaeth hon, bydd angen i Hywel Dda ffocysu mwy o’i adnoddau ar wasanaethau lleol sy’n cael eu cyflenwi gan dimoedd gofal sylfaenol, cymunedol a gofal cymdeithasol a’r sector gwirfoddol (neu drydydd sector). Bydd cyflenwi system iechyd sy’n ffocysu ar ofal yn agosach at y cartref yn gofyn am gefnogaeth poblogaeth Sir Gaerfyrddin, Ceredigion a Sir Benfro, yn ogystal â rhanddeiliaid. Y prif ffactor sy’n sbarduno newid yw’r cyfle i wella ansawdd a diogelwch gwasanaethau iechyd.

Cynllun Ymgysylltu a Chyfathrebu
The Health Board is committed to working in partnership with all stakeholders to ensure we can deliver world class quality care.
A significant engagement process was started in August with more than 40 staff events held and a range of stakeholder events, including the Stakeholder Reference Group, meetings with the Community Health Council and voluntary sector events. Doctors are also working with us to develop proposals through expert ‘Think Tanks’. Presentations have been given to numerous partners, including Local Authorities, Local Service Boards and Health, Social Care and Wellbeing Partnerships, and letters offering presentations have been sent to hundreds of community groups recently.
The Health Board is also communicating with the public through newsletters and further engagement activities will take place over the coming months.
