Who we are
Conwy Intermediate Care Service (C.I.C.S) refers to a range of services provided in partnership with Conwy Social Services Department, the Welsh Ambulance Services Trust, Leisure Services, Health Providers and the Voluntary Sector. The service is aimed at preventing avoidable hospital admission or enabling timely discharge in order to promote independence and maximise recovery.
Services may be delivered in the patient’s residence and can be supported by Intermediate Care beds in

What we do
A multi-disciplinary/agency team provides the service which is time limited to a maximum of six weeks, sometimes as little as two or three days. The patient is assessed as soon as a referral has been received from either the patient’s GP or other health/social care professionals. A care plan is completed and may include various treatments according to individual needs such as physiotherapy, occupational therapy, nursing intervention and/or social care support.
Intermediate care is a now a model of excellence within Conwy and is much more than a team of health and social care professionals. It is this approach that provides rapid response, enablement, rehabilitation, admission avoidance and accelerated discharge services for all adults in Conwy that need such provision.
