Sickness Certificates

For any illness lasting seven days or less, patients should obtain form SC2 (Self Certificate) from his or her employer. Medical sickness certificates for an illness lasting longer than seven days will be provided by your GP. Certificates issued for less than seven days involve non-NHS work and will attract a charge.
