Sick Note

When do I need a sick note?

Self-certification

If you consider that you are too ill to work you can sign yourself off work for the first 7 days using a Self Certificate form obtainable from your employer or you may print off this form by following this link Self Certification Form

If you are off work for more than 7 days your doctor can provide an NHS certificate following a consultation. If your employer requires a certificate for the first 7 days a private certificate can be issued. Please note that a fee is payable for all private sick notes.

Freedom of information act

From 1 January 2005 you can ask for access to general information held by the practice. Your personal health information will continue to come under the Data Protection Act 1998.This act aims to promote greater openness and accountability across the public sector, including the NHS and independent contractors i.e. General Practitioners.

GP's are legally obliged to adopt and maintain a publication scheme. Please ask the Practice Manager for Ystwyth Medical Group's publication scheme, which is based on the model suggested by the Welsh Assembly Government.
