What is the Individual Health Record?


The Individual Health Record (IHR) is a summary of your GP record, which will let GPs and nurses working in your local Out of Hours Service see some basic information about your medical history. 

Without this essential information the medical staff providing care when your GP surgery is closed at evenings and weekends, only know what you tell them. Yet, when you are unwell it's sometimes easy to forget details of any medication or allergies.

The record that is being introduced in Wales does not form part of the English electronic summary care health record. In Wales, we have a different approach.
