Interest in and attention to chronic conditions management has grown considerably in the last five years, but have services and care improved? Has a focus on prevention worked? What have we learnt so far?
This conference provides a unique opportunity to take stock of international developments to date, challenge assumptions and inform next steps. The conference itself draws on leading international expertise as well as the experience of those working directly with people and patients to prevent illness, improve services and find sustainable and affordable solutions for the future.
The aim of the conference is to stimulate debate and provide inspiration for the challenges ahead, highlighting developments and future opportunities to improve chronic conditions management and public health.
